Health surcharge payment FLR (FP)

Good morning all,
Pls I want to pay health surcharge for FLR (FP) but I can’t find that specific option. Pls what do I click for this FP payment. Pls help urgently.

Thanks Moderator for your response, but actually what I wanted to do is , when I tried paying for the FLR (FP) I wasn’t seeing that particular option I saw FLR (O) and some other ones , so I got confused because I want to make sure I don’t tick the wrong option . Pls advice . It’s urgent . Thanks
