ILR main applicat and dependent

Hi
I am currently in Tier 2 general visa and will be completing 5 yrs in April and planning to apply for ILR same day service. I believe I have to you SET (O) form.
what all documents I should submit?

My dependent is not eligible for ILR at present, He is in UK for past 2.5 yrs.
which application form I have to use?
which application form i should use - FLR (O) or FLR(IR)?
Need to spend another 5 yrs? this past 2.5 yrs wont count?
Please advise.

3 Months payslips proving you earn the requirement threshold of £35,000pa.

Tier 2 Dependent. There is no need to switch visas to another category.

Need to spend another 5 yrs? this past 2.5 yrs wont count?

If you switch to another visa category, the 5 years will start again from scratch.

If I don't switch visa for my dependent then it will expire on Feb 2019. The total time he spend in UK will be 3 yrs. Is there any way that i could extend his visa without starting from the scratch?

Also I read that, if I chnage from Tier 2 general to ILR -- Then dependents has to apply for visa within 20 days of my application??

If i want to extend the dependent visa after i applied for ILR. what application form should I use to apply for extension?

Tier 2 General Dependent - it is an online form.
