biometric residence permit (Lost or Stolen) Help

Hey every one hope ur ok just a quick help need i got my Permanent Residence BRP in Oct and lost it two days ago somebody took it from the car with my wallet, anyway i try to look for it for two days but was unable to find it, so i report HO and police today and Home Office replied with email that they canceled it and i av to apply for replacement which will cost £56, problem is i cant see anywhere do i need to send all documents which i sent b4 or i just need to send application form, can someone plz tell me which form do i need to fill and what documents i need to send, bank, job, payslips, NI etc

i applied using same link and fourm and after a month i got my application back and they said i used inappropriate application form, and they telling me i was previously issued residence card non eea family member under the eu route, i sent covering letter that its not RC card this application is for PR card which was lost i sent the copy of lost PR but i dnt know why they dnt read, can i appeal or send application again.
