Apply for tier 2 dependent visa

Hi,
My husband has a tier 2 visa expiring soon and would like to apply for dependent visa under my tier 2 general visa( granted for 5 years and sponsored by A rated sponsor). He will finish his 10 years in UK in 2020 .
1) will we have to apply and pay for IHS for 5 years or can we apply for 2 years? Or is it safe to apply for 5 years?
2) do we have to show proof of maintenance?
3) what are the documents required?
4) what is the additional fee for 1 day service?

Thank you for your reply. I have got an ISA help to buy account in which the funds were maintained. Will I be able to use that to show my accounts or will it have to show my normal bank account statements ?
