About my wife tier2 depandent visa extended

Hi everyone.My wife visa is due to expired soon!Please can I get some advice and information To extend my wife visa!I am now a British citizen,I applied tier2 general skilled worker when I came to uk,So my wife came here as a tier2 depandent(which is family member)My question is:
1.Where can I download the paper application form?
2.When can I submit the application form?Is 3 months before the visa is due or 1 month before the visa is due?

Can anyone of you explain to me please!If you can,Can you forward the link where I can find the information,And how can I download the paper application!

2. Between 28 days prior to expiry and before the expiry date. Don't apply too early, if there's an issue with completing the qualifying period for ILR under 319E.

There is no paper form for extensions. It is online only.
