Maiden name and married name

Hi,
I'm in the process of filling in my AN application. I'm not sure whether I should fill in point 1.7 in the application. I'm married and all my documents such as passport, ID are in my married name but 2 years ago I wasn't married yet so I was using my maiden name. Should I put my maiden name in 1.7 or leave it blank? Is it only to do with your first name (of it was different at birth) not your surname? Thanks for your help.

You need to state your maiden name in 1.7. as it is different to the one you have now.
Char (CR001 not Casa)

Thanks for your reply. That's what I thought but thebguy that was helping me with the application left it blank so I wasn't sure.
