spouse change of name Tier 2 General (5 years)

I am failing to under the forms that on the uk gov immigration website. My wife recently changed her name on the passport to my name. Which form should i use to notify this change? If it is TOC form can she apply by herself without including me and our son since she is the one who has a name change? or the need all they everyone's details. How much should i pay ? how does this form work. I will be happy if someone can explain how this form works..she has a tier 2 dependant visa. If you have gone through the process people give me more details of the process
