ILR for Tier 2 dependent

I am about to apply for the ILR for my wife and wondering if you are in a similar position as me and guide me with my application process please?

My position: I completed my five(5) years of continuous stay in the UK last April (April 2017) and moved from Tier-2 General to ILR. However at the time my wife, who was on Tier-2 dependent (PBS dependent) visa, had just completed four (4) years of continuous stay in the UK. So we did not change her visa status and she continues to be on Tier-2 dependent visa as of date.

My Question: Since my wife will complete five(5) years of continuous stay in April 2018, and she's on Tier-2 dependent (PBS dependent spouse) visa while I am on ILR, what is the correct application form type? Is it form SET(O) or SET(M) or FLR(M)?

Any response will truly be appreciated.

Set(O) for PBS Dependents.
