Queries on SET (O) Tier 2 General (5 years) and dependent wife

Queries on SET (O) Tier 2 General (5 years) and dependent wife

Hi,
Me and my wife have my PEO appointment in soulihull in 2 days. We both are applying at the same time. I have completed 5 years on Tier 2 general (Student visa and PSW for 2.5 years before) and my wife also completed 5 years as Tier 2 dependent (psw dependent before).
I have the following queries on SET (O) form. I am applying in one SET (O) form showing my wife as my dependent.

1)Page 7 - payment details - should be left blank as I have already paid for PEO appointment?

2) On page 13 which category should I only tick Tier 2 migrant or tick tier 2 dependent as well?

3) On page 15 for english language - I have a UK degree - should I only tick the following?
The qualification was taught or researched in the UK or a majority speaking english countty (other than canada)

4)Page 18 - what should I fill for number of hours this salary is based on - is it calculating the working hours in the whole year or I can specify per annum?

5) On page 20 for 6.1 which date I should fill in - my PEO appointment date for my first Tier 2 general (approved on same day) or my from date on first BRP for tier 2 general(there is 2 days difference for me compared to the PEO appointment date) or my first date in UK on student visa?

6) how is 6.11 (days out side UK and crown dependencies) different to 6.2 (days outside UK)?
DO I need to fill in both 6.11 and 6.2 both for the same days outside UK?

7) My wife got last tier 2 visa in Jun 2017. Should she mention no in 6.14 (have you had any absences from the UK during any leave granted since 11 january 2018) and leave the table blank below (for filling days outside UK)?

8)11.2.4 - country of passport issue and issuing authority - my passport was issued in UK thoough indian. Should I fill united kingdom and republic of india?

9) On page 78 - for the question ' for period before you were granted leave as a tir 2 migrant, documents showing that you met the relevant requirements of the immigration rules' - do I need to tick this (as I had pws and student visa before tier 2 which was before 5years) and provide the psw approval letter and student visa stamp on the passport?

Based on HOs SOC list and the information it contains.

Thanks a ton CR001. Just a little clarification needed for the country and issuing authority for passport. I got my passport in Birmingham and it says on the passport stamp that consulate general India.

Should I then mention India Consulate General of India in the answer box?
