When to apply for Tier 2 extension?

I have been offered a job in the Uk and the employer applied for my Tier 2 VISA through a visa specialist. They applied for a 3 years instead of 5 years (i understand duration is based on the Cos).
Can someone tell me why was it done for 3 years instead of 5?

Also, if i and my employer decides to extend my VISA, when should it be done? Can we do it right away or should we wait until to the last couple of months till my VISA expires?

How would we know why this was done??? Have you asked your sponsor???

You apply for an extension shortly before your visa expires.
