ICT to high earner visa above £159600

IF I move from ICT to high earner tier 2 visa for salary above £159600.

1. Will there be a cooling period applicable for me to switch to Tier 2 high earner from ICT?

2. Can I apply for my new visa from UK only? will this come under CoS or RCoS? will this come under monthly limit?

3. Will there be a cooling period once I finish my tenure on high earner visa?
