Reference check for kids in MN1 form

Hello Dear,

As mentioned in MN1 form, first reference should be a professional who has engaged with child in a professional capacity, such as teacher, health visitor etc. I asked my son’s teacher to sign and she did not give her Date of Birth while have school address in address field. As per her, she never gave DOB and gives school address only.

Would it work?
