Set O Form Online application - Premium appointment

Apologies, if this was already posted by someone.

I am filling online application Set O form for my wife (from Tier 2 Dependant to ILR). However, I can't find anywhere a link to book premium appointment. I have filled almost all the sections of the Online form, except Declaration and Payments section of the form.

Can some one please tell me how/ where to find the link to book appointment at Premium centres for the above category. ?
