ILR Premium application Not with in 28 days of qualifying period

I am a Tier 2 general visa holding applicant having 5 years continuous employment above the threshold salary(for the last 3 years), UK masters and Life in UK test passed. My Tier 2 visa started on 1st July 2013. So I can apply for an ILR now.

I am about to submit my ILR Settlement application using premium service (appointments available after second week of Aug only at Croydon) I have the following queries -

there is a clause Premium application appointment must be within 28 days of you completing your qualifying period but no appointment with in that period, so what should I do? Can I submit my application using premium service?
If I select options Settlement - SET (O) is that right - for Category Tier 2 (General to ILR)?

The 'within 28 days' is the EARLIEST you can apply. You can apply at any time even if you have reached or passed the 5 years residence.

Set(O) is correct.

thanks for the clarification. I thought I can't submit premium application after 28 days, POST option only available!!
By the way

1. 6 months ago, I was in maternity leave and started to work from Jan '18 after taking 9 months of maternity leave . Do I need to submit the details of that?

2. My salary was below the new threshold rule during the first two years and after that it is above the level for the last 3 years- is that OK?

1. I don't believe so but probably safer to have the information with you.

2. Irrelevant as long as it was in line with your SOC.
