Filling AN Form Application for Naturalisation

Hello,
Trust, you guys are fine. Its nearly one year after ILR and I am filling my AN Form Application for Naturalisation and have few things to clear please,
A) Q1.5 State your name as it appears on your passport: Surname/Family name?
Shall I put only my surname ( not the first name)

B) Q1.22 Knowledge of language/life in the UK.
I have done my Msc here in UK and LIUK test . its asking for the test number which number they asked(I think LIUK ) am I right?

C) Q1.32 Details of your partner.
I got married after my ILR and my wife never been in UK , will apply for her after few months.Shall I have to give all details of her?

D) Q1.48 Address of employer or business.
The company in which I work heaving many contracts in different cities within UK. And I am working in one of them sites. Our main office is in different city. And In P60 the main office address is shown not the site where I work. What shall I write in it, the place and the city where I am working or the main office address?

E) Q 6.7 BRP enclosed.
I am going with NCS, to keep my original documents with me. its asking for location. Is it ok to say "Keeping with me" ?

F) Q 6.12 If the required BRP is not enclosed give details why you are unable to provide it;
What should be the good answer for this, please.

Hi there,
I am in a situation, when I passed my LIUK test I was on Entrepreneur visa and have had used my BRP as a form of id and it was kept by home office when I got my ILR BRP. I have heard we have to attached the form of id which we have had used while taking LIUK test. will that be issue or is it ok, please?

That great, thank you very much CR001.One more confusion, please In Payment slip there is a column of HO Ref: What to write in it, please? Is it AN for Naturalisation and UKM, UKF for Registration (Ceremony fee) or some thing else ?

The professional referee can be a person over 25, on the list and be of any nationality but must have known you for 3 years.

The other referee must be over 25 and be British and must have known you for 3 years.

Both must be of good character.

While reviewing my application ; under Employment history , I am working for my current employer for very long time but since I am employed the business (company ) has changed name 3 times, So, got confuse here what to do , to split my employments dates or keep it under once name( the current name), please. What should I do here , shed some light, please.
