tier 2 locum job

I am currently on tier 2 visa working in NHS in a permanent position. The director has asked me to fill a position of locum consultant for one year (December 2018 to December 2019).This requires a new tier 2 visa as job code will be changed. I have promised job at the end of this locum. I am due for ILR in February of 2020 I have the following questions

1. Can I work a full time locum consultant on tier 2 visa as hospital need me to work as locum consultant with regular terms of jobs (not through agency locum )as i cannot work as regular consultant as I don't have CCT so hospital accommodating me as locum consultant
2. If my job title locum consultant would effect my ILR visa which is due in 18 month
