Two amendments required on COS - Urgent Help Required!!

URGENT HELP!!

I need help one below matter

I am on tier 2 G visa expiring on 30th Oct 2018. My employer has assigned me unrestricted COS for extension but I have noticed two mistakes:

1. My current home address on COS is as my address on my passport which is address in India. They used my passport to assign COS hence missed to use my local address.

Can my correct address be amended through sponsor notes?

2. They have entered below start date and end date

Start date: 29th Oct 2018
End date 28th Oct 2020

Can they change end date through sponsor notes so that my extension is for three years instead of two years?

I would be extremely grateful if some one can share knowledge based on their experience.

I am bit nervous about above two amendment which needs to be made on my assigned COS.

Why have you registered a second username to ask the exact same question again when you have already been given the answer after posting with your other username? Multiple usernames are NOT permitted on the forum at all.
