Tier 2 Visa Extension Refused because i did not meet the minimum salary requirement

Hi,
i need some advice on my visa application.

i recently applied to extend my visa, SOC code 2123. However my application was refused because i did not meet the minimum salary requirement. My current visa (BRP) expires on the 12/11/2018, thats roughly 3 weeks from now. My current salary is £29,999. Before i re-apply again i have a couple questions

1. Do i require a new COS from my employer in-order to make a new application ?
2. can the salary stated on my current COS be amended to the appropriate amount by my employer and can i then re-use the same COS in my new application ?
3. Do i have to make a new application within 14 days from the refusal date of my first application ?
4. What is the appropriate salary for my job code 2123 based on a 37.5 hour week. The immigration rules appendix J states £35,200. Is this figure based on a 37.5, 39 or 40 Hour week.

1. You need a new CoS

2. You need a new Cos with the correct salary.

3. You must apply before your current visa expires.

4. You need to work it out. The appendix j salary is likely based on 39 hours.
