ILR premium service for PBS dependent--URGENT

Hi

I have arrived in UK as Tier 1 General Dependent on 3rd august 2013 and had my visa extended as PBS dependant on 6th February 2016 on this extension I have been granted three years visa which will be expiring in Feb 2019. After this extension my daughter was granted British Nationality within a month. My partner also become British national last year.

I have managed to book Premium service appointment for this week end and have following questions.

-I have now read on Premium service portal (https://visas-immigration.service.gov.u ... nt-booking)that:
"If you are applying for settlement, your premium service appointment must be within 28 days of you completing your qualifying period"

My questions are

1) Since my 5 year stay in UK was completed on 3rd of August 2018 does this mean I cannot apply for ILR using premium service?

2) Premium service portal mentioned I have to download a form which i have to download and bring it for my appointment but I am unable to locate any downloadable SET(O) form on UKBA portal. Seems like at start of November they replaced the downloadable pdf form with online version only. But if you try to fill in the online form, at the end it only shows existing and new service which both take 26 weeks for a decision. Is premium service disabled for SET(O) now? If not where can I find the downloadable form?

3)In the online form, what do you reply to "Supporting documents explaining why you are applying for leave to remain". I am a PBS dependant now, what documents should I provide?

4)Also for "Evidence of your immigration status" and documents supporting current visa, should BRP be applicable for both these document requirements?

1. You can use the premium service. Within 28 days is the soonest you could apply OR before your current visa expires.

4. Brp is enough.
