SET(O) Dependent application Consent & Declaration

Hi,

Main applicant ILR is approved. We have submitted SET O dependent and child application.

Can some one clarify below documents please.

Declaration signed by Mrs XXXXX (Main applicant name) to confirm consent for the Home Office to request verification checks-- Does Part1 need to be signed by main applicant or dependent applicant?

Consent form for the Home Office to request verification checks on Mrs XXXXXX(Main Applicant name) for joint accounts or third party support -- Hope this is Part 2 signed by main applicant ?

Do we need provide two part 1 signed by dependent applicant and main applicant or only dependent applicant?
Cohabitation -- i am providing 2 proofs for last 2 years (council tax joint, broadband bill - main applicant & gas bill --dependent applicant)-- will this sufficient? or need more.
