She can apply before visa expiry and book her appointment after eligibility date and she will make up for the shortfall via section 3C automatic leave extension. As long as she is eligible on the date of the decision she will be granted ILR. No need for an extension

The date of your application is 30th August and you will be protected by section 3C when your visa expires as your application was done in-time

Was the appeal in 2012 filed in-time ??
Was the new application in April 2014 applied within 28 days of court of appeal rejection ?
If yes to both, then she can apply under long residence

I haven't seen any case at very present but definitely seen a lot cases few years ago when UKBA (UKVI's previous name) used to return the application as invalid if someone is not eligible for the applied route on the date of application/paid incorrect fee/used wrong application form/didnt sign. It is not a risk at all. You should be eligible on the day of the decision. That is because when you apply and visa expires, section 3c kicks in. So HO cannot refuse you given you would be eligible.

A CCJ older than 6 years is wiped from the official records. They have no effect on ILR

3 different sources with both your names is ideal but you can send more evidence showing you lived together in the same address during the same period.
