In 2012 march i got Portugal Nationality through my father, i Came to UK in June 2012 and got a job same month and in September i sponsered my wife as EEA dependent she got EEA permit came to UK in October 2012

Now we both have permanent residence and eligible to apply for British Citizenship in October as i was filling online form it has been asked date of arrival in UK so which date should i mention the one which i entered on student visa that is june 2006 or 2012 EEA entry date.

I belive this is ththe date when you first entered UK. This the date I put in my application. So for you it will be 2006 and for your wife 2008. I am happy to be corrected by experienced member.

very very wrong advise , as per guide lines you are supposed to write down the date you entered UK with the view of long term residency...

i came as a student in 2006 and went back in 2009
came back again in 2013 and applied for AN recently .. i read guidelines thoroughly for this question.

Section 2: Residence requirements
2.1 Enter the day you first arrived with a view to staying in the UK on a long-term
basis, and the airport or seaport through which you then entered. If this is less than 5
years before the date on which we receive the application, or less than 3 years
before this date if you are married to or the civil partner of a British citizen, you might
not meet the residence requirement (see page 6 of the Booklet AN) and your
application may be unsuccessful.

It's simple. If you are applying as an EU citizen you need to use 2012. I am also happy to be corrected if I am wrong.

An active registered member of the NMC?

https://www.nmc.org.uk/

If yes, then no issue to use her as long as she has known you for 3 years.

I am in chaos, i tried everywhere and asked everyone i know professional, no one is ready to give their passport number i dont understand why they make silly excuses.

Can i give both references but not 1 professional but a British passport holder and write a letter explaining that i cant find any professional reference will Home Office except this

Your professional referee can be a person of any nationality but they must have known you for 3 years.

Have you looked at the list to see if there is a friend who meets the requirement of a professional referee??

It is their choice if the don't want to be a referee. You can't force anyone.

Don't you have any friends who are directors of their own limited company??

Religious leader etc??

The details get submitted with your application you submit, as entered online, the Referee Declarations don't have any of their details on, they just need complete your full names on page 1, their full names, date and signature on page 2. Of course, your photo will be on page 1.
